Ngā mōhiohio e pā ana ki tēnei mahi (About this position)
The Older Persons’ Mental Health Community team provides psychiatric and geriatric care for the older members of our community. We aspire to provide high quality care to patients in the community with mental health needs and/or complex age related health conditions. We work in partnership with our patients, their families, the clinical team and community carers and supports to achieve the best possible health outcome for the patient.
The Clinical Nurse Coordinator (CNC) will work in partnership with the Nurse Manager to provide operational and clinical leadership and to facilitate high quality, safe and effective client-focused care. The CNC has responsibility for the day to day running of the community service, including triaging of referrals, facilitating admissions and clinical oversight of a multi-disciplinary team. The OPMH community service is based at Alexandra Hospital; however the role includes coordination for Wairau based service.
This role will include the following:
- Provide specialist nursing advice, support and expertise
- Improve the experience for consumers, including their family and whanau
- Develop effective relationships with partners in care including primary health and Aged Care sector
- Ensure that quality systems are in place across the service
- Challenge and identify barriers to service effectiveness and efficiency and work with key stakeholders to implement changes, to provide more effective and timely care/interventions.

You will be a Senior Registered Nurse with mental health and/or older persons’ experience. Ideally you will have a strong leadership background and be able to lead the team through periods of change and uncertainty.
You will act as a role model for contemporary mental health nursing and professional nursing practice and will be held in high-regard by your peers. You will be passionate about equity and service delivery and committed to ongoing development and further enhancing the capacity of your team.
For the health and safety of staff and patients all NMH staff are required to be vaccinated in accordance with the COVID-19 Public Health Response (Vaccinations) Order 2021.
